Who will be my home health care provider?
Mary Free Bed has teamed with Residential Home Health to create Mary Free Bed at Home. This joint venture allows the two health care providers to work closely together for smooth transitions from hospital to home, and to continue meeting the highest standards of care.
You have the right to choose your home health care provider. Medicare rules and regulations state that patients who are leaving the hospital and need home health care receive a list of all available home health agencies certified by Medicare.
How soon after I leave the hospital will my home health care begin?
Mary Free Bed at Home nurses and therapists are available to provide care 7 days a week. Although several factors can affect the specific timing, we aim to make our first visit within 24 hours of when you leave the hospital.
What should I expect during my first home health care visit?
Your Mary Free Bed at Home nurse or therapist will work with you and your caregivers to check your head-to-toe health status, review your medications and consider home safety concerns. This care professional will spend focused one-on-one time with you to help you set and achieve personal goals for health and independence. You will also receive a patient packet, which has important information and paperwork you can refer to throughout your care.
What is Mary Free Bed at Home’s service area?
Mary Free Bed at Home provides care in western Michigan counties including: Oceana, Newaygo, Mecosta, Muskegon, Montcalm, Ottawa, Kent, Ionia, Allegan, Barry, Van Buren, Kalamazoo, Berrien, Cass and St. Joseph.
What does home health care cost?
Medicare covers 100 percent of eligible home health care services, with no out-of-pocket costs. The following conditions must be met for eligibility:
- Home health care is ordered by your physician
• The home health care agency is Medicare-certified
• You are homebound, meaning you’re unable to leave home without assistance (with limited exceptions), or it’s not recommended for you to leave home
